The American Association of Heart Failure Nurses (AAHFN) would like to thank you for your interest in our certification exam. Please be sure to read all of the eligibility requirements, review all of the application instructions and all of the other sections of the Certification Center thoroughly before starting to complete the application. The completed application must be submitted with the correct fees in order to be reviewed.

AAHFN is NOT responsible to contact candidates whose applications are incomplete. AAHFN will attempt to contact those with incomplete applications but it is ultimately the candidates’ responsibility to make sure their application is complete. Incomplete applications will still be charged the application processing fee of $50.00.

Please complete the application form with the name you wish to see on your certificate upon passing the exam. The address that you provide on the application is the address that your test results and certificate will be sent. There will be an additional charge if either of these documents (test results/certificate) needs to be re-sent.

Within 2-4 weeks of receiving your application, AAHFN will confirm if your application has been accepted. You are advised to keep a copy of your completed application for your records. AAHFN is not responsible for correspondence lost in the mail.
